IEHP’s Health Homes pilot program initially was called the BHICCI - Behavioral Health Integration and Complex Care Initiative.
The idea behind this pilot was to take their most complex patients - patients with behavioral health and chronic physical health issues and give them a single place where their care needs could be met. The goal was to reduce the friction that was inherent in the current way of providing care to create better outcomes for these patients and their communities.
